Instructions for Online or Mail in Application

Online Application Process

When completing the online application, parts of the application are completed in an online form and part to be downloaded (PDFs), filled out, saved to your hard drive, and then attached to your email to us. Step 1: Complete our Online Database Form;

(IMPORTANT: First, download the fillable pdfs below to your hard drive, then fill them out and send it to Admissions. DO NOT FILL THEM OUT ONLINE as they will not save your information–they must be downloaded prior to filling it out)
Step 2: Parents complete the Parent Questionnaire (fillable pdf);
Step 3: Student Applicants complete the Student Questionnaire (fillable pdf) and 600-750-word essay;
Step 4: Request Letter of Recommendation from a current Math and Humanities/English teacher (email fillable pdf to your teachers); print your name on both forms and email to your teachers:
Mathematics Teacher Recommendation Form;
Humanities/English Teacher Recommendation Form;
Step 5: Parents fill-out and return the two Release of Records forms:
Release of Records, Last Two Years;
Release of Records, Cum File Request;
Step 6: Optional: Parents are invited to include a note commenting on the nature and personality of their student;
Step 7: Submit all completed materials to admissions@summerfieldwaldorf.org, along with:
-copies of the last two years of your student’s school records, for quicker processing, and;
-$95 Application Fee ($150 for International Students). Pay online (by ACH* or Credit Card; link below), or in person. 
*ACH is the preferred method of payment so your entire fee goes to the school rather than a portion being paid to the credit card companies.

Step 1: Print out the Application document, which includes:
-the Parent Questionnaire;
-the Student Questionnaire (includes instructions for a 600-750 word essay);
-recommendation forms for a current Math and Humanities/English teacher;
-two Release of Records forms;
Step 2: Fill out all forms;
Step 3: Optional: Parents are invited to include a note commenting on the nature and personality of their student;
Step 4: Submit to the Admissions Office (655 Willowside Rd, Santa Rosa, CA 95401):
-all completed application documents;
-a photo of your student;
-copies of the last two years of your student’s school records, (to expedite the process), and;
