Immunizations
Effective 9/1/2016, all teachers and volunteers in ECE must be immunized against influenza, pertussis, and measles by state law. Please read the following document for how to satisfy these requirements.
You may waive the influenza vaccine annually and may use this form to do so, or you may write your own note: Flu Waiver
Mandated Reporter Training
Just in case you missed it on the General Hiring Paperwork page, Mandated Reporter Training is required specifically for Child Care Providers. The link below will take you to the training website. When you have completed the mandatory training, provide me a copy of your Certificate of Completion, either electronically or hard copy.

In Addition…
In addition to the above paperwork, you will have to complete a Live Scan for the Dept. of Social Services (in addition to the Live Scan required by the School.)
Should you prefer to go to CONCENTRA MEDICAL CLINIC for your physical and TB Risk Assessment and/or Test, you may do so; Concentra will bill us directly for their services.
